Upload Insurance Card & Immunization Records

In order to participate in camp the following forms are required of each participant:

• Copy of your insurance card
• Immunization record which  includes:
  • 2 MMR vaccines
  • 1 Tdap vaccine within the last 10 years
  • 2 varicella vaccines or proof of having had chicken pox

This immunization record can be a copy from your personal file or from the physician’s office. If you have questions regarding the immunizations please call the Midland Asst. Football Coach – Ryan Williams at 612-723-1202.  Students who are unable to produce these records will be held from participation until records are produced.

INSURANCE CARD CAN BE SUBMITTED BELOW BY FOLLOWING THESE STEPS:

STEP 1: Enter Your Registration ID Number - From Your Receipt
STEP 2: Enter Camper's Last Name - From Your Receipt
STEP 3: Click Submit
STEP 4: Click Upload, Select Proper File - On Your Computer, Click Open
STEP 5: Form is Successfully Uploaded - Status Will Indicate the Uploaded Date
